Filling out a sick list allows the indication in the column "Organization name" of both the full and short name of the employer. If the constituent documents do not provide a short title, and the full exceeds 29 characters (as many cells are allocated on the letterhead of incapacity for work), an arbitrary reduction is permissible: the registration number will help the FSS to correctly identify the insurer's organization. In addition to the name of the organization, the employer enters the employee's IDN and SNILS in the sick list, notes the conditions for calculating the amount of benefits and length of service, the period for which the allowance is calculated, and information for calculating the allowance and the total amount.

Since 2013, the rules for filling the sick list have undergone some changes: for example, contact with the borders of cells of letters and numbers is no longer a violation. In the event that a mistake has crept into the registration of the sick list, and it turned out to be spoiled, a new blank sheet is not issued, you need to make a duplicate. In order to receive a duplicate, the employee must first prove that he has not yet received the benefit on the disability sheet .
