A medical policy is a document that attests to the citizen's right to receive free medical assistance to the extent that is determined by the compulsory medical insurance program . It allows you to apply for help to doctors in any part of the country, regardless of the place of registration. The document must be issued free of charge. It is issued at work or at policy points. The document must be kept by the insured citizen.
A medical policy is issued for a certain period, moreover, one document is issued to only one person. It is not necessary in cases where urgent assistance is required. The document is presented only with a passport. If the policy is lost or its owner has changed the place of residence (name), it must be exchanged or a duplicate order ordered. Thanks to this decision, everyone can count on the qualified help of medical personnel, on respect from him, on the possibility of choosing a doctor depending on their preferences.
Now there is a reform of the healthcare sphere, so the old-style medical policy is to be replaced. Now a document with a single form will be used. The exchange procedure is based on the application of a citizen who wants to receive a new document.
Replacement of the medical policy on the application of not the insured person himself, but his representative is also possible. It is necessary to present personal documents of this representative, as well as a power of attorney, which allows you to carry out actions on behalf of the person receiving the new document. The application must be filled in manually. The form is issued directly at the point where the documents were submitted. You can submit them personally, by contacting the insurance department, or send via the Internet. In this case, you need to obtain a document confirming that the application was accepted, as well as a temporary certificate that allows you to use the insurance system. It will operate only 30 days from the date of application, however, during this period you must already issue a new medical policy.
The procedure for re-issuing a document or obtaining a duplicate must be made in such cases:
- Change of personal data (place of residence, surname);
- a bad state of the policy, in which it is impossible to read the data;
- loss of the document.
Within a set time, you will receive a medical policy of a new format. You will be informed of the date of receipt by telephone or e-mail. Along with the new document, you should receive a memo, which will indicate your rights and responsibilities, as well as the duties of medical workers.
