A little bit about illnesses

Initially, it should be noted that the Murphy reaction is a symptom that is most often an indicator of a disease such as cholecystitis (an inflammatory process in the gallbladder). However, it can also occur with inflammation of appendicitis. Again, the diagnosis is made after palpation of a specific area on the human body, as a result of which the patient reacts in a special way to pressing the doctor.

What is Murphy's symptom?

It is also necessary to understand the term itself. This reaction is named after the scientist Murphy. This symptom is manifested as a result of the doctor's actions, that is, palpation of a certain area of the body.

  1. With cholecystitis, the doctor pushes a little in the area of the right hypochondrium of the patient. The hand of the doctor should be placed so that the thumb was just below the costal arch, that is, approximately in the region of the location of the gallbladder. The remaining fingers are located along the edge of the costal arch. The doctor asks the patient to take a deep breath. If it is painful or impossible - you can say that Murphy's symptom is positive. That is, a person has suspicions of cholecystitis.
  2. If we are talking about appendicitis and even started peritonitis, there will be a blunting of percussion sound from the right side of the body.

Speaking about the symptom of Murphy, it should also be noted that this indicator is not always manifested in patients with cholecystitis. This happens only in about a third of patients. That is why it is important to note that it is unreasonable to diagnose on the basis of only one symptom. Moreover, this is a very specific indicator.

Nonclassical way to determine the symptom of Murphy

In medical dictionaries, you can also find a slightly different way of determining the disease according to Murphy's symptom. In this case, the patient does not need to lie down on his back. So, the doctor will need to wrap the patient's trunk with his left hand so that the thumb is located in the right hypochondrium. Then the patient will have to exhale, as a result of which the finger will have to plunge deeper into the body. Then I'll have to inhale. If at the moment painful sensations arise, doctors are inclined to say that Murphy's symptom is positive. But in this case it should be noted that this way of clarifying the positivity of the reaction is applicable to small people or children. With large-bodied patients, this method of diagnosis will be completely irrelevant.

A few words about the symptom of Ortner

This is also a symptom that can indicate a problem solely as a result of pain. This diagnostic method is based on the fringing of the edge of the costal arch of the patient (on the right side). If at the moment there are painful sensations, the symptom of Ortner is considered positive. But there is also a test moment. So, for accuracy, it is necessary to carry out similar manipulations on the left side of the body. If there is pain here, it's not about cholecystitis. We must look for a different problem. Alternatively, as an option, make sure the veracity of the patient's words.

Other similar symptoms for problems with the gallbladder

Nobody will argue that Murphy's symptom in cholecystitis is not the only indicator of this disease. So, it is important to take into account other nuances. It is interesting to note that there are quite a lot of so called bubble symptoms. These are the indicators bearing the names of such scientists as Vasilenko (the symptom is useful in the early stages of the disease), Kera (painfulness on inspiration), Mussi-Georgievsky (tenderness in palpation of the muscle), Rysman (needling at the edges of the costal arch with respiratory arrest), Boas And Lepin.
