Glomerulonephritis - treatment of acute and chronic forms of the disease

If the patient is diagnosed with acute glomerulonephritis, the treatment of the disease is carried out under the conditions of a therapeutic hospital. In the hospital department the patient is for one to two months. The duration of therapy depends on the course of the disease, the presence of complications. Out-patient treatment is continued for four months, its duration is the best prevention of transformation of acute glomerulonephritis into chronic.

Diet therapy should be designed for a long time, so you need to take into account the shape, flow, stage of the disease. Her requirements for glomerulonephritis are most consistent with Pevzner's diet No. 7. Patients with glomerulonephritis without impaired renal function should receive an average of 1 gram of protein per kilogram of body weight. With the appearance and progression of symptoms of the disturbance of the nitrogen excretory function of the kidneys, the amount of protein in the diets is gradually reduced from 40 to 20 grams, depending on the severity of chronic renal failure.

Pathogenetic treatment includes complex therapy with the use of antibiotics, antihypertensive drugs, diuretics, glucocorticoids, immunosuppressants, anti-inflammatory drugs, anticoagulants and antiplatelet agents.

Antibiotics are used for 5-7 days. Diuretics are prescribed for fluid retention, increased blood pressure and the development of heart failure. If swelling is absent, but there is hypertension or the effect of diuretics is insufficient, prescribe antihypertensive drugs. Glucocorticoids have anti-inflammatory, desensitizing and immunosuppressive effect, increase diuresis, reduce swelling and manifestations of urinary syndrome. The purpose of these drugs is indicated in the protracted course of glomerulonephritis. With severe puffiness and a significant decrease in diuresis, anticoagulants are prescribed, which improve microcirculation in the kidneys and reduce inflammation.

After inpatient treatment, the patients are on clinical follow-up for three years. At this time, it is contraindicated to perform physical work, stay in cold and wet rooms.

Undiagnosed acute glomerulonephritis, the treatment of which has not been carried out, goes into a chronic form. The methods of treatment of chronic glomerulonephritis are chosen depending on the clinical course of the disease, the features of morphological changes in the kidney, the progression of the process and complications. Treatment of chronic glomerulonephritis should continue for a long time, often throughout the life of the patient. Adherence to diet, diet is mandatory for all patients, whereas drug treatment is not shown to everyone.

Patients diagnosed with chronic nephritis and glomerulonephritis should avoid hypothermia, physical and mental overstrain. Categorically prohibited work at night. Once a year, planned in-patient treatment is conducted. If the patient has chronic glomerulonephritis, treatment of colds should be performed with the mandatory appointment of bed rest and antibacterial drugs.

During the exacerbation of the disease, corticosteroids are used. With the development of hypertonic and nephrotic forms, hypotensive and diuretics are prescribed . Antiplatelet agents, cytostatics, immunosuppressants, anticoagulants are shown. With a form such as uncomplicated chronic glomerulonephritis, treatment often leads to the development of persistent remission. Also, sanatorium treatment in conditions of dry hot climate is shown.
