Modern wound healing ointments.

In connection with the change in the structure of pathogens of purulent wounds, there was a need to use effective means of a new generation. Obsolete wound-healing ointments do not suppress the infection at the proper level. The use of ointments such as Ichthyol, Vishnevsky, Gentamycin, Streptocide, is illegal in modern medicine. These drugs do not have the necessary antimicrobial ability. They do not anesthetize and do not relieve swelling.

A thorough treatment of wounds by surgical means involves the treatment of injuries. The wound should be "tightened" in the shortest possible time. This will significantly reduce the period of therapy and get an effective result. However, the majority of cases of surgical intervention in the healing process of injuries require medical treatment taking into account the wound process phase. Therefore, the selection of an agent that suppresses the pathogen of a purulent process is very important. The wound healing ointments should have an active necrolytic action, a prolonged osmotic activity. In addition, the property of drugs aimed at slowing the drying of the wound surface is needed to stimulate the growth of granulations that contribute to the formation of the epithelium.

Modern clinical practice of treatment of purulent wounds includes wound healing preparations on a polyethylene oxide base. The development is based on the use of a combination of polyethylene oxides with different molecular weights. The new generation of ointments introduced a number of antimicrobial components: levomycetin, dioxidin, metronidazole with levomitsetinom, iodine with polyvinylpyrrolidone, furacillin, hinifuril. In addition, wound healing ointments contain trimecaine, which has an analgesic effect; Methyluracil. The latter has anabolic and anti-catabolic activity aimed at the process of formation of new cells in exchange for damaged cells.

The wound healing ointments made on the PEO-base differ from the obsolete means by a multifunctional action. The osmotic effect has a duration of up to 18 hours. This allows you to make dressings no more than once a day. Another important advantage of preparations on a polyethylene oxide base is extensive antimicrobial activity. It should be noted the drug "Mafenide-acetate 10%". It is made on a hydrophilic basis and suppresses in the wounds gram-negative bacteria.

Despite the frequent use of ointments based on levomycetin and dioxin, the high antimicrobial activity of these drugs persists for more than 20 years. Technological development of new means for wound healing is based on the use of nitrofuran compounds. To such means concern: "Ointment hinifurila 0,5%", "Furagel". New wound healing ointments of domestic production with nitrofuran compounds showed high clinical activity and antibacterial effect.

In the treatment of wounds with fungal infection, "Iodopyron Ointment 1%" is used. It has significant bacteriological activity. It is indicated for patients with burned extensive wounds, bedsores, trophic ulcers. In our time, multicomponent, iodine-containing ointments are produced abroad. They are widely used in clinical practice. These are such preparations as: "Aegis" (Hungary), "Zorka form" (Yugoslavia).

The antimicrobial, osmotic activity of ointments on the PEO-base allows to stop the development of purulent process in the first four to five days after tissue damage. For the treatment of anaerobic infection, preparations containing nitazole are used. Their action is aimed at combating streptococci, E. coli, spore-forming bacteria, pathogenic anaerobic microorganisms. On the basis of nitazol, preparations have been developed: "Streptonitol", "Nitacid".

The wound healing ointment of the new generation has the ability to arrest the development of various bacteria. It localizes multiple infections at any stage of the wound process. Timely application of the necessary funds will allow the elimination of purulent inflammation in a short time, and also to prevent it. The use of wound healing properties with antimicrobial components significantly reduces the duration of treatment of patients.
